Easy four step process
1. Connect with our team
Call us or email us to let us know you are interested. We are happy to answer any questions and help you determine if Big Gym Therapy is a good fit.
2. Complete online forms
You will receive an email from our office inviting you to join our secure patient portal.
Join the portal and complete and sign our online forms.
3. Provide prescription
Obtain a prescription from your doctor authorizing physical therapy or occupational therapy services.
4. Get on the schedule
Once your online forms are completed and insurance is verified, we will contact you to get your child on the schedule.
Payments & Insurance
Before beginning therapy, we will verify your insurance benefits and review your coverage with you.
For your convenience, Big Gym Therapy will submit your insurance claims for you. You are responsible for any amount not covered by your insurance plan.
